The Twelve Clans Unity Hospital’s Health Information Management Department is responsible for the creation of, care, storage and retrieval of patient medical records. It also provides statistical information for the medical and administrative staff.
Patients may request medical records, immunization records, etc., by calling (402) 878-2231, Ext. 2208.
Patients and/or their guardians can also access medical records by signing up for a Personal Health Record account. The instructions to do so are featured below.
